In November 2019 the US Department of Justice announced a hospital system and a group of cardiovascular surgeons agreed to pay $46 million dollars to resolve allegations they submitted slightly tainted bills to the Medicare program. One of the allegations that Medicare paid inflated prices for the medical treatments/surgeries, allegedly they also billed Medicare more than once for the same procedure, and the hospital gave the medical doctor (friends) great deals on rent and additional other goodies. No one went to jail.
